What Work Remains When an Accreditation Submission Deadline Is Close?
Reviewed October 2026
In IHS's reading, sort each open item into one of four groups: can make now, cannot make in the time left, depends on an outside party, or retrieval only. Whether you can have more time depends on your accrediting body's own program documents. None of the documents reviewed for this page says when asking for more time is the better choice than holding to the date, so this page makes no recommendation either way.
Check your program's documents on extensions and list what depends on the current date: your accreditation's expiration, any Medicare deemed status, and any state license or payer contract that relies on your accreditation.
Records must be real. Do not fabricate activity, backdate approvals, or turn a planned meeting into completed minutes.
Which deadline and program should we check first?
First identify whether the date is an application deadline, an evidence cutoff, or a survey date, then confirm the services and locations in scope.
CARF's application captures service scope, locations and a requested two-month survey window. On site, CARF surveyors watch services being delivered and talk with persons served. NABP's Community Pharmacy review covers the licenses held by the pharmacy and its pharmacists, its policies and procedures, and an on-site visit announced in advance. CARF's steps page also says getting ready can take a year or more.
Which of our evidence can we still make, and which can we not?
In IHS's reading, when several departments owe documents, the four groups apply as follows: policies, plans and training not yet held can be made now; logs, reports and minutes for a past period cannot be made in the time left; third-party agreements depend on an outside party; and records that already exist are retrieval only. IHS's sort below uses ACHC checklist 703 as its example. The checklist lists the items an ACHC surveyor reviews at the survey; it is not an application. It covers Fitter, Home/Durable Medical Equipment, Medical Supply Provider, and Complex Rehabilitation & Assistive Technology Supplier services only.
Can make now.
The policy manual, with each policy tied to the ACHC standard it addresses; a written education plan; and any training session not yet held. For the grievance and complaint item, the checklist names a plan to enact the standard once accredited as the alternative to a log. Record each policy's actual approval and effective dates; a new policy proves only its own approval date.
Cannot make in the time left.
Logs, reports, ongoing-education evidence and committee minutes for a past period need real activity from that period; activity that can still happen before the date belongs under "can make now." For a renewal applicant partway through a three-year accreditation cycle, the items the checklist marks with an asterisk, incident logs among them, call for three years of records.
Depends on an outside party.
Third-party agreements. When another person or organization delivers care for you or shares responsibility for it, ACHC checklist 703 lists both the contract and that party's professional liability insurance certificate. A counterparty controls its own review and signature, so an internally approved draft stays an outside dependency until the other side acts. The checklist also puts the three-year renewal asterisk on the item covering how you oversee contracted providers, so a signed agreement does not cover that oversight history.
Retrieval only.
Whatever already exists: a kept incident log, minutes of a meeting held, attendance records, the executed agreement and the insurance certificate. Locate the executed version before commissioning a replacement. Separate records held elsewhere from activity that never happened.
What are our options for the work we cannot finish in time?
In IHS's reading, if every open item is a document or class, ask whether each can be finished, approved and in place by the date; when an item needs operating history you lack, the paths are to continue toward the date and disclose the gap, move the date, or submit in a later cycle. Check your accreditation agreement and the standards edition that applies to your organization for any terms on deadlines, extensions and evidence windows; this page describes none for URAC. This page does not establish that any program accepts a disclosed gap or a later-cycle submission.
CARF's 2026 Continuing Care Retirement Community manual asks for a reasoned written extension request before the resurvey application is timely filed, approves none until that application is submitted, and warns that late filing risks a lapse. CARF's other manuals were not reviewed. ACHC's Specialty Pharmacy (revised 2023) and DMEPOS (revised 2021) process documents allow a written request to postpone a survey; ACHC's refusal process replaces postponement the day before an announced survey, or on the day of an unannounced one. Once ACHC accepts a postponement, you must pay a fee before rescheduling and have 180 days, counted from the postponement date, to confirm in writing that you are ready for survey; after that, ACHC may keep the deposit and treat the application as forfeited, and you would have to apply again. NABP's Application Guide, which shows no publication date, describes no extension process. For DMEPOS accreditation, the Board of Certification/Accreditation's (BOC) Standards Guide (updated May 15, 2026) says that no accrediting organization, BOC included, may push back the date on which an existing accreditation expires. The Joint Commission's public process page says nothing about postponing a survey.
In IHS's reading, retrieval comes first and shows gaps; send outside-party requests at once; draft last.
What can we do about unsigned minutes and unfinished training?
In IHS's reading, first determine whether the committee met and whether a genuine record exists; an absent signature and an absent meeting are different gaps.
Where a governing body applies, ACHC checklist 703 lists a year of its meeting minutes among the survey items. This checklist is silent on a general rule for accepting late signatures.
Where your applicable requirements call for signed minutes, record the actual signature date. If no meeting occurred, hold the needed meeting prospectively and identify the historical gap.
In IHS's reading, a class held now can be finished before the date; it does not supply earlier periods. ACHC checklist 703 lists a written education plan together with proof of ongoing education, and the renewal asterisk sits on the proof only.
Could a stated exception change the answer?
In IHS's reading, yes, but only within the exception's stated scope.
For checklist 703's DRX6-3B-G item, ACHC allows initial applicants to provide collection tools for later data capture instead of completed PI activities.
NCQA's FAQ dated January 15, 2026 covers a narrow set of elements: UM 2, Element B in Health Plan and Behavioral Health Accreditation, and UM 4, Element B in UM Accreditation. Only for surveys in the 2026 standards year (July 1, 2026 through June 30, 2027), an organization may submit a detailed implementation plan for electronic point-of-care access to UM criteria, with a timeline that finishes by June 30, 2027. The same FAQ shortens the look-back for those elements in full, on renewal surveys as well as first surveys: the six-month period is gone, and the test is whether the requirement is met before the survey date.
What this is NOT
This is preparation before submission or survey. It does not address an accreditation-denial appeal or a rejected plan of correction.
This is not legal advice. Take questions about legal duties, record corrections or contractual consequences to counsel.
This is not a readiness determination, a promise of additional time, or a prediction of any accreditor's decision. It does not establish URAC requirements or transfer one program's evidence windows to another.

Frequently asked questions
Can a consultant finish the missing records for us?
Not a record of activity that never happened: that cannot be created, and IHS does not try. IHS provides gap assessment, evidence mapping and drafting for your reviewers to approve. If your team can do the sort above and close the open items itself, outside help is optional.
Who asks about an unresolved requirement or changed date?
Your organization sends the question and any submission. IHS can draft the question and organize supporting material. IHS never contacts, submits to, confirms with, or represents a client before an accreditor, regulator or payer.
