Can We Get an Extension on an Accreditation Deadline?

Reviewed October 2026

In the programs read, the accreditor decides whether a date can move and how late you can ask: ACHC's pharmacy and DMEPOS documents (2023, 2021) apply the refusal process from the day before an announced survey; CARF's 2026 CCRC manual makes extensions of accreditation discretionary.

Before you ask, identify the deadline, establish who can change it, check what depends on that date, including what losing your current accreditation would trigger, and set the completion date you would propose. NABP's and BOC's DMEPOS surveys are unannounced, and accrediting organizations with CMS-approved deeming programs agree, under 42 CFR 488.5(a)(4)(i), to re-survey through unannounced surveys, so, in IHS's reading, you may have no announced survey date to move. Under CARF's 2026 CCRC manual, an extension is usually no longer than three months. The Joint Commission's public accreditation process page is silent on postponing a survey, and this page says so wherever a document is silent.

IHS drafts, and the client sends.

Which deadline are we actually asking to move?

In IHS's reading, moving a survey date and getting more time to submit are separate requests: ACHC's 2023 and 2021 documents take a survey postponement in writing, while NABP's two application documents describe no extension procedure. NABP's documents run two clocks: an accreditation or inspection application still unsubmitted or unpaid 60 days after it is created is deleted for good, and one submitted short of information is canceled if what NABP asks for does not arrive within 30 days.

Write down the triggering event, due date, program and governing document for each deadline, record any existing accreditation expiration separately, and ask which date your change would move.

Who can approve more time, and what has the body published?

Start with the accreditor's process document or your assigned contact. The ACHC documents, covering Specialty Pharmacy and durable medical equipment, prosthetics, orthotics, and supplies (DMEPOS), say a postponement is requested in writing to the Account Advisor, that acceptance brings a fee due before rescheduling and 180 days from postponement to confirm readiness in writing or risk forfeiting the deposit and application and having to reapply, and that from the day before an announced survey, or the day of an unannounced one, the refusal process applies instead.

NABP's Application Guide lists DMEPOS among its unannounced on-site surveys, and BOC's May 2026 Standards Guide says its DMEPOS surveys are unannounced and not pre-scheduled unless CMS approved the supplier's exception request in writing before its accreditation expires. NCQA's 2022 online application guide has the organization request survey dates that NCQA approves and does not describe changing a date once approved. The NABP pages reviewed publish no rule on rescheduling a DMEPOS survey, on how late to ask, or on the effect of a late survey on an existing accreditation, and the two public application documents publish no extension procedure; that silence does not show NABP never allows extensions. The URAC documents were not reviewed for this page, so confirm in your URAC agreement and the notice that set your date whether URAC allows a date to move, how late to ask, and whether a fee applies.

CARF's Level of Care certification conditions concern extending an expiring certification term, not a submission deadline. Under condition 7, the written request, with its reasons, comes ahead of the recertification application; CARF will not approve one until the application is filed and the fees are fully paid; and three months is the usual ceiling. CARF's 2026 CCRC manual likewise asks for the written request before the resurvey application is filed, and treats its scheduling notice as final, with a fee for later date changes. These are Level of Care and CCRC terms; CARF's other manuals were not reviewed.

What still depends on the original date?

For nonexempt DMEPOS suppliers, CMS instructs contractors to revoke billing privileges when required accreditation is not maintained. For Medicare deemed status, the CMS State Operations Manual says a terminated or withdrawn accreditation removes it unless another CMS-approved accreditation is in place.

In IHS's reading, an extended evidence deadline alone does not show that Medicare billing stays protected. Read your accreditation agreement for your expiration date and renewal terms, then each state rule and customer contract that depends on continuous accreditation, and note whether it refers to submission, an award, or continuous accredited status.

Check when your accreditation expires, the survey date, and what the accreditor's document says about a survey after expiration. ACHC's Specialty Pharmacy process document (revised 2023) says a renewal survey before expiration continues the term from the old expiration date, but under Accreditation Pending or Dependent Status an acceptable plan of correction, and any Dependent Survey, must also come before expiration. A later survey starts the term from the last survey day or, on those tracks, that plan's receipt. BOC's DMEPOS Standards Guide says no accrediting organization can extend an existing DMEPOS accreditation's expiration date. ACHC's January 2026 DMEPOS flyer says DMEPOS reaccreditation becomes annual once a current three-year term expires, so a pharmacy with DMEPOS codes should confirm its term and cycle in writing before asking to move a survey.

Would the requested time solve the problem?

Request time only against a completion plan showing what the delay will accomplish.

If NABP cancels an application, or you withdraw it, expect a handling fee; refunds follow that program's own policy, which the Application Guide does not state.

What should our written request contain?

In IHS's reading, the following is a drafting framework, not an accreditor's prescribed form.

First, identify the organization, program, application reference, deadline notice, the date you want changed and the proposed replacement date. Second, explain the cause of delay with supporting records, separating completed work from remaining evidence. Third, ask for a written decision identifying the approved scope, conditions, and effect on existing status. For a CARF Level of Care certification, the organization itself can ask CARF ahead of filing how a request is likely to be received; the conditions invite that early contact, and IHS does not make it on the client's behalf.

What should we do while the request is pending?

Keep a plan for the original deadline while preparing for approval or refusal. In IHS's reading, silence is not a basis for treating extra time as granted. Under CARF's Level of Care conditions, missing the application due date with any piece of the package, whether the application, the extension request or the fees, can leave the certification to lapse.

Those conditions measure the next three-year term from the prior expiration date as though no extension had been granted, which in IHS's reading means the extra months come out of the next cycle.

This page does not recommend withdrawal; obtain the applicable terms before choosing.

What this is NOT

This is not legal advice. Counsel should interpret disputed contract terms, state licensing obligations, and consequences for operating or billing.

This is not an appeal of an adverse decision. If you already have a denial, use the accreditation denial appeal page to frame that different problem.

This is not a promise of approval or a universal extension period. The CARF and NABP examples are program-specific, and the ACHC terms come from dated documents that may have changed. No reader-specific deadline or entitlement is established here.

Frequently asked questions

Do we need a consultant to request an extension?

You may not need a consultant if your team can identify the governing procedure and build a supported completion plan. IHS can draft the request for your review; the client sends it. IHS does not contact, submit to, or represent clients before accreditors, regulators, or payers.

Can a consultant submit our NABP application for us?

No. NABP's Application Guide limits the account and the submission to the applicant's own staff, and an outside consultant or temp does not qualify. IHS drafts materials for the client's review and submission.
