QUAD A Accreditation Fees: What the Schedule Says

QUAD A publishes one fee schedule for its outpatient, pediatric dentistry, Medicare surgical, physical therapy, rural health clinic and international programs, and it prices each in a different way. The figures below come from the schedule PDF that QUAD A links from its prospective facilities page, opened October 3, 2026. IHS's QUAD A accreditation page covers the programs and the survey process, and this page lays out the full tables.

What QUAD A publishes and where

The schedule is a 10-page PDF at quada.org/hubfs/QA-3-2024-Fee-Schedule-v4.pdf. The file name carries "v4" and the document prints no other version number. It states that the fees apply to all invoices as of April 7, 2025.

IHS looked for a newer schedule on quada.org's program, prospective facility, FAQ, office-based surgery and standards news pages on October 3, 2026. The fee schedule links on the prospective facility and office-based surgery pages point to this same PDF, and the other pages list no other schedule, so it is the version used here. Verify current fees with QUAD A.

Non-Medicare programs (office-based surgery and procedural, and related programs)

Each row's annual fee depends on the physician count, the number of specialties and the facility class. The schedule's header counts physicians, dentists or diagnostic personnel toward that number. The Full Survey fee does not vary with size or class: $2,560.

Each row's annual fee depends on the dentist count and the facility class. The survey fee is $2,560 whatever the size or class.

How the fee structure works

Two charges make up most of what a facility pays. The annual fee follows the size of the clinical staff, and for most programs the number of specialties and the facility class. The survey fee is flat for non-Medicare programs and rises with facility size for ASCs.

The class is set by the anesthesia a facility uses. QUAD A's anesthesia class requirements document, opened October 3, 2026 (its pages are marked "Effective 02-02-2026"), places local anesthesia and minimal sedation in Class A, moderate sedation in Class B, and deeper techniques such as propofol, spinal, epidural and general anesthesia in Class C.

Plan for the two fees to arrive together on a single invoice once every three years, in the re-survey year, about six months before the facility's accreditation expires. Hiring or losing physicians or clinical staff during the term can change the annual fee.

An organization with 15 or more accredited facilities gets a discount on the annual fee, from 13% at 15 to 49 facilities up to 25% at 150 or more, and QUAD A resets the tier each quarter. The schedule also lists 10% discounts for members of certain associations in some programs, and those do not stack with the volume discount.

Reading the schedule

This is arithmetic on the published figures. It is not a quote, an estimate of any bill, or a prediction of what QUAD A will invoice a particular facility.

The schedule does not explain how the annual fee is invoiced in the two years between surveys, whether the class is assigned at application or at survey, or how a facility's size is measured for the ASC table. Ask QUAD A before budgeting.

Is the survey fee the same for every facility?

For non-Medicare programs and pediatric dentistry, the Full Survey fee is $2,560 for any size or class. For Medicare ASCs it is $4,130 for small, $5,480 for medium and $6,280 for large facilities, and a $3,135 Life Safety Code fee is listed in addition.

What decides which class column applies?

The class follows the anesthesia and sedation a facility uses, as set out in QUAD A's anesthesia class requirements document. Class A covers local anesthesia and minimal sedation, and Classes B and C cover deeper levels. Confirm your class with QUAD A before you budget.

Is there a fee for an expedited survey?

A procedural, surgical, oral and maxillofacial surgery or pediatric dentistry facility can ask for a survey within 30 days of submitting its completed materials. The schedule charges $550 for the request, and QUAD A also passes on its surveyor, travel and lodging costs. Facilities in Medicare programs cannot request an expedited survey.

What happens to the fee if a facility cancels a survey?

A written cancellation 31 or more days before the survey date costs nothing extra. Within 30 days the schedule lists a $550 fee plus QUAD A's other cancellation costs, and a cancellation after the surveyor has traveled, or within a week of the survey, adds a further survey fee. The schedule treats changing a survey date as a cancellation. Verify current fees with QUAD A.

Does IHS charge or collect QUAD A fees?

No. QUAD A invoices the facility directly, and the facility pays QUAD A. IHS scopes each engagement after a free introductory call.
