ACR Accreditation Fees by Modality and Unit Count

ACR charges accreditation fees by modality, and inside each modality by the number of units and modules applied for. This page tables the schedule in ACR's fee article, from CT and MRI to mammography and radiation oncology. Every figure is ACR's published schedule as we read it on October 3, 2026, and none of it is an IHS price. Verify current fees with ACR before you budget.

Last reviewed: October 2026. ACR fee article opened October 3, 2026.

What ACR publishes and where

The fee article sits on ACR's accreditation support site as one page: a table by modality, a paragraph on radiation oncology and a revision history (ACR, Fees and Payments, page opened October 3, 2026). The article links no separate PDF of the schedule. The page title carries the date 5-6-2026, the page shows a last-modified stamp of August 4, 2026, and the revision history for 5-6-2026 records updated CT module fees. The article states no effective date for the figures, so the dates above are the only dating the source offers.

An older address ending in "revised-4-2-2024" served the same article when we opened it on October 3, 2026 (older ACR address). Verify current fees with ACR.

Replacement certificate, all modalities except mammography, then mammography$50, $65

How the fee structure works

Each modality has its own rows. CT, MRI, nuclear medicine, PET and mammography price a first unit and a lower additional unit at the same geographic location. The gap is $100 for CT and MRI, $250 for mammography and a wide one for nuclear medicine and PET, where an additional unit costs $750 per module.

Module count moves the fee in different steps. CT and MRI hold at $3,200 through four modules, then add $200 per module. Nuclear medicine and PET add $700 per module on the first unit.

Repeat rows and mid-cycle rows are separate charges in ACR's table, and the article does not say when each one applies, so ask ACR before you count on one. ACR gives a 10 percent discount to a site that applies in three or more modalities, and mammography does not count toward those three. The discount comes off that site's total, and ACR's online application figures it for you. A large practice can raise volume pricing with ACR through a support ticket.

ACR's process article defines a practice site as each geographic location where imaging is performed. Seven modalities go through one application program: breast ultrasound, CT, MRI, nuclear medicine, PET, stereotactic breast biopsy and ultrasound. Mammography and radiation oncology each need their own application (ACR, The Accreditation Process, page opened October 3, 2026).

What the schedule does not include

Reading the schedule

Arithmetic on ACR's published figures, not a quote or an estimate of any bill:

  1. One site, two CT units, each at up to four modules: $3,200 plus $3,100 is $6,300.
  2. One site, MRI and CT at up to four modules and nuclear medicine at one module, one unit each: $3,200 plus $3,200 plus $2,300 is $8,700. Ten percent is $870, leaving $7,830. ACR's online system calculates the real figure.
  3. One site, mammography (first unit, one module), MRI and CT: $2,000 plus $3,200 plus $3,200 is $8,400. IHS's reading, October 3, 2026: mammography does not count toward the three modalities, so no discount applies. Confirm the count with ACR.

Where this fits

This page covers ACR fees only. Our diagnostic imaging accreditation readiness page covers what imaging accreditation is and how IHS builds the requirements matrix and evidence index. The diagnostic imaging comparison matrix sets ACR, IAC, RadSite and The Joint Commission side by side without fee amounts. What this is not

Frequently asked questions

How much does ACR charge to accredit a CT or MRI unit?

In ACR's fee article titled 5-6-2026, the first unit applying for up to four modules is $3,200, and the fee rises $200 per module from five to seven. Each additional unit at the same geographic location is $100 lower at every module count. Verify current fees with ACR.

Is there a discount for accrediting several modalities at one site?

Yes, for a site that applies in three or more modalities, ACR takes 10 percent off that site's total, and mammography does not count toward the three. The article says the discount also applies to the renewal fees of the other modalities at that site.

What does ACR charge for radiation oncology accreditation?

The article lists $9,500 for the main facility and $3,000 for each additional site. Verify current fees with ACR.

Are phantom costs and refunds part of the fee?

Some modalities use a phantom, and its cost is a separate payment to the phantom's maker, not to ACR. The article prints no phantom price. ACR also states that paid fees are not returned and that the figures can change.

Does the ACR fee article give an effective date for these figures?

No. The article carries the date 5-6-2026 in its title and a last-modified stamp of August 4, 2026. Its revision history for 5-6-2026 records updated CT module fees. Ask ACR which date governs an application you are about to file.

Does this schedule include FDA mammography inspection fees?

The article does not mention inspection fees. It lists ACR's accreditation fees only, and it tags the mammography fees as approved by the FDA. For MQSA certification, see our mammography and MQSA page.
